Kindly download or print, fill-out and sign our credit application. We only issue open lines of credit to businesses and encourage that you include a list of references with your application. By filling out the credit application you represent yourself as a municipality, government agency or Dun and Bradstreet rated corporation.
Once you've completed the application, please email it to firstname.lastname@example.org or fax it to 818-936-0645. Your application will be processed within 1-2 weeks. Please note the application must be signed to receive consideration and there may be additional information required in order to open a line of credit. If you have any further questions, please e-mail our credit department.
Orders: We require a Purchase Order to be emailed to process your order.
Terms: 2% 10 Days Net 30 days from the date of shipment.
Limit: The max limit given is $10,000, it is subject to approval.
